    TLDR Intralesional corticosteroids and 5-alpha-reductase inhibitors are the best treatments for frontal fibrosing alopecia.
    A systematic review of 50 studies involving 1,478 patients found that intralesional corticosteroid injections and 5-alpha-reductase inhibitors (finasteride/dutasteride) are the most effective treatments for frontal fibrosing alopecia (FFA). Oral isotretinoin shows promise for improving facial papules but has minimal impact on hair regrowth or stabilization of hairline recession. Topical calcineurin inhibitors are useful in combination therapies, while oral antimalarial drugs and systemic corticosteroids are less effective. Emerging treatments like tildrakizumab, oral minoxidil, and PRP injections show potential but require further clinical trials. Hair transplantation offers temporary satisfaction but long-term results are disappointing.
