Comparative Analysis of the Clinical-Pathological and Immunopathological Characteristics of Lichen Planopilaris and Frontal Fibrosing Alopecia

    TLDR Lichen planopilaris and frontal fibrosing alopecia are likely the same disease with different clinical appearances.
    The study compared the clinical, histopathological, and immunopathological features of lichen planopilaris (LPP) and frontal fibrosing alopecia (FFA) in 20 patients (10 with LPP and 10 with FFA). Clinically, FFA presented with symmetrical frontotemporal recession and thinning eyebrows, while LPP showed multifocal scalp involvement. Histopathologically, both conditions exhibited similar features, including perifollicular lymphocytic infiltrate and basal layer destruction. The study concluded that despite clinical differences, LPP and FFA are histologically and immunologically indistinguishable, suggesting they are different clinical manifestations of the same disease.
