TLDR Macrophages help activate hair follicle stem cells, affecting hair growth and skin repair.
The study demonstrated that skin-resident macrophages played a role in the activation of hair follicle stem cells during the murine hair cycle by undergoing apoptosis before stem cell activation. This process was associated with changes in gene expression, including Wnt transcription. By inducing macrophage apoptosis in early telogen, researchers identified macrophages as contributors to stem cell activation in vivo. Additionally, inhibiting macrophage-specific Wnt production delayed hair follicle growth, suggesting that perifollicular macrophages provided a novel regulatory cue for the regenerative activity of skin epithelial stem cells. These findings had potential implications for skin repair, inflammatory skin diseases, and cancer.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

Prostaglandin balance affects hair loss, particularly in conditions like Lichen Planopilaris, where an imbalance can lead to hair follicle damage. Treatments mentioned include prostaglandin analogs and Pioglitazone HCL, with a focus on maintaining prostaglandin equilibrium for potential hair regrowth.

Clinical studies by Dr. Barghouthi and Dr. Bloxham indicate that Verteporfin, when used with FUE and FUT hair transplantation methods, shows promise in hair follicle regeneration and minimal scarring due to its ability to inhibit Yes-associated protein (YAP). Microneedling at depths of 3-3.5mm, combined with Verteporfin, could potentially reactivate dormant follicles, although the optimal dosage and application method are still under investigation. Concerns remain about the DHT sensitivity of regenerated follicles, highlighting the need for further research to optimize trauma levels and Verteporfin concentrations to achieve effective and scar-free hair regeneration.

Lichen Planopilaris (LPP) is an autoimmune condition causing permanent hair loss and fibrosis, often misdiagnosed. Treatments include pioglitazone, topical corticosteroids, anti-inflammatory medication, and Jak inhibitors.
