TLDR Adipose tissue-derived exosomes may help treat lichen planopilaris and preserve hair.
This case report describes a 50-year-old woman with treatment-resistant Lichen Planopilaris (LPP) who experienced significant improvement after a single session of adipose tissue-derived exosome therapy. Her LPPAI score decreased from 7.83 to 1.83 over six months, with reduced symptoms and visible hair regrowth, particularly in the central parietal zone, without any adverse effects. The findings suggest that exosome therapy could be a promising treatment for LPP, but larger studies are needed to confirm its efficacy and safety, and to compare it with other regenerative therapies. Limitations of the report include the lack of histopathological images, follow-up biopsy, and photographic documentation.
