TLDR Platelet-rich plasma (PRP) is an effective treatment for lichen planopilaris.
A single-blinded randomized clinical trial involving 24 patients with lichen planopilaris (LPP) compared the effects of platelet-rich plasma (PRP) injections combined with topical clobetasol versus topical clobetasol alone. Both treatments initially reduced the Lichen Planopilaris Activity Index (LPPAI), but after 6 months, the LPPAI significantly increased in the clobetasol-only group. The PRP group showed significantly lower LPPAI scores at 2 and 6 months and higher patient satisfaction at the end of the study. No serious adverse effects were reported, and PRP injections were well-tolerated. PRP is suggested as a new and effective treatment option for LPP.
