This case series highlights the importance of comprehensive scalp examination in diagnosing concurrent lichen planopilaris (LPP) and female androgenic alopecia (FAGA) in individuals with skin of color. The study involved three cases, where trichoscopy and biopsies were used to differentiate between FAGA and LPP. Findings showed that while the temporal scalp exhibited signs of FAGA, the frontal scalp showed characteristics of scarring alopecia indicative of LPP. All patients were treated with clobetasol 0.05% solution and minoxidil 5% foam/solution for FAGA, and intralesional triamcinolone acetonide for LPP. The report underscores the necessity of total scalp dermoscopy and targeted biopsies for accurate diagnosis and treatment planning.
