TLDR Minoxidil works faster, but ketoconazole is a safer alternative for female hair loss.

The study compared the effectiveness of topical ketoconazole 2% and minoxidil 2% in treating female pattern hair loss (FPHL) in 40 patients. Both treatments were effective, but minoxidil showed earlier clinical improvement while ketoconazole showed a delayed response. The frequency of treatment-related side effects was higher in the minoxidil group. The study suggests that topical ketoconazole 2% can be a safe and effective alternative to minoxidil for treating FPHL. Further studies are recommended to evaluate the efficacy of higher concentrations of ketoconazole solutions for FPHL.
