A review of 5-alpha-reductase inhibitors found approved topical minoxidil and oral finasteride remain preferred over newer agents. Oral minoxidil 5 mg was not superior to topical 5% twice daily overall, though it looked better on the vertex and half developed extra body hair. Adding stromal vascular fraction to PRP gave no significant benefit over PRP alone, so cheaper PRP should come first. Platelet-rich fibrin showed significant effects in five clinical studies, but preparation protocols lack consensus. Necrosulfonamide in mice promoted hair growth and countered DHT-induced growth inhibition. Tofacitinib and baricitinib achieved 53% response in severe alopecia areata with 40% reporting adverse events, none requiring hospitalization. JAK inhibitors were safe and efficacious for moderate-to-severe alopecia areata, though discontinuation often causes relapse.
