The study examined the relationship between the 2019 EULAR/ACR classification criteria and organ damage in 562 patients with systemic lupus erythematosus (SLE). Patients were divided into two groups based on their EULAR/ACR scores: group 1 (scores ≥20) and group 2 (scores 10–19). Group 1 exhibited a higher prevalence of symptoms such as fever, alopecia, and renal disease, and had significantly higher organ damage scores, particularly in the renal, cardiovascular, dermatological, and gonadal domains. The renal domain of the EULAR/ACR criteria showed the strongest correlation with renal damage. The findings suggest that the EULAR/ACR criteria not only aid in disease classification but also have potential utility in predicting organ damage in SLE patients.

Histopathology and serology indicated possible neutrophilic urticarial dermatosis (NUD) or hypocomplementemic urticarial vasculitis syndrome (HUVS). The patient later experienced central nervous system vasculitis, treated successfully with methylprednisolone, intravenous immunoglobulin, and plasma exchange. This case underscores the importance of considering serious systemic diseases in patients with nonpruritic urticated rashes and highlights the need for comprehensive serological testing and biopsy for accurate diagnosis and early treatment.
