A 39-year-old Japanese woman with severe alopecia areata (AA) and atopic dermatitis (AD) showed no improvement with conventional treatments or excimer laser (EL) alone. However, combining 0.5% delgocitinib ointment with EL resulted in complete hair regrowth within 2 months and full remission after 1 year. This case suggests that the combination of delgocitinib ointment and EL may be an effective and safer alternative to oral JAK inhibitors for treating severe AA, especially in patients with AD. Further studies are needed to understand the mechanisms and evaluate the therapeutic synergy.
