    TLDR Topical immunotherapy is the best treatment for severe alopecia areata.
    The article reviewed the use of topical immunotherapy with diphenylcyclopropenone (DPCP) or squaric acid dibutylester (SADBE) as preferred treatments for severe alopecia areata, especially when over 50% of the scalp was affected. It covered the safety and efficacy of these treatments, advancements in their application for pediatric patients, the molecular mechanisms behind their effectiveness, and improvements in safety through targeted delivery methods.
