Effectiveness of Oral Tofacitinib Treatment on Patients With Moderate-to-Severe Alopecia Areata in Iran

    TLDR Tofacitinib is effective and has minor, reversible side effects for treating severe hair loss in Iran.
    The study involved 47 patients with moderate-to-severe alopecia areata who were treated with oral tofacitinib, a Janus kinase inhibitor, for at least 6 months. After 18 months, hair loss decreased by 6.45 times and body hair loss decreased four times compared to the start of treatment. Only one patient experienced sensory-peripheral neuropathy, which recovered after treatment cessation. No other complications were observed. The study also found that alopecia areata patients had higher hair loss on the head, body, and nail involvement, especially those with other autoimmune diseases like vitiligo and hypothyroidism. The average maintenance dose was 7 mg daily, reducing the risk of side effects. The study concluded that tofacitinib is recommended for treating alopecia areata in Iran due to its effectiveness and minor, reversible side effects.

      A woman who has been experiencing hair loss for several years, and her question of whether there is any benefit to getting a biopsy to check if it's AGA or diffuse alopecia areata when no cure or very effective treatment exists. Treatments such as spironolactone and Minoxidil/finasteride/RU58841 have previously been discussed.
