Keratosis follicularis spinulosa decalvans (KFSD) was a rare, X-linked disorder characterized by scarring alopecia of the scalp and eyebrows, often accompanied by widespread keratosis pilaris. The condition was sometimes associated with ocular abnormalities and palmoplantar keratoderma. This report documented a case of KFSD with acne keloidalis nuchae and tufted hair folliculitis, further supporting the association between these conditions. Previously, acne keloidalis nuchae had been described in only one other patient with KFSD, highlighting the rarity and complexity of these overlapping dermatological conditions.
