Keratosis Follicularis Spinulosa Decalvans And Acne Keloidalis Nuchae

    TLDR A man with scalp and neck skin issues improved after a year of oral isotretinoin.
    A 27-year-old man had a 10-year history of scarring alopecia on the scalp, follicular crusting, pustule formation, and a papular eruption on the neck, along with keratosis pilaris on the cheeks, eyebrows, and thighs. Histology revealed scarring with mixed perifollicular inflammatory infiltrate and acute suppurative folliculitis. He was diagnosed with keratosis follicularis spinulosa decalvans and acne keloidalis nuchae, a previously unreported association. The patient responded well to a 12-month treatment with oral isotretinoin 20 mg daily.
