    This case report details a 35-year-old woman with extensive follicular porokeratosis (FPK) leading to alopecia, associated with type 2 diabetes mellitus. FPK, a rare condition characterized by keratotic papules and spicules, was confirmed through dermoscopy and histopathological examination. Despite treatment with oral isotretinoin and topical tacrolimus, the progression to scarring alopecia was not prevented. The report underscores the potential link between FPK and diabetes and suggests that FPK should be considered in differential diagnoses of keratotic papules and alopecia.
