    Punctate follicular porokeratosis was identified as a distinct clinical entity characterized by cornoid lamellae originating exclusively from hair follicles without associated annular plaques. The second reported case involved a 56-year-old African American female with keratotic lesions on her right posterior shoulder. Histopathology confirmed the presence of cornoid lamellae within hair follicles, supporting the diagnosis. This condition's diagnosis relied on proper histopathologic sampling to reveal the punctate follicular cornoid lamellae.
