The joint position paper by various Polish medical societies provides a new definition and management guidelines for metabolic syndrome (MetS), a health issue characterized by obesity, hypertension, and disordered carbohydrate and lipid metabolism. The new definition includes obesity and two of the following criteria: high blood pressure, impaired glucose metabolism, or elevated non-HDL cholesterol level. The paper emphasizes lifestyle modifications as key interventions for prevention and treatment of MetS, but also suggests medical management, including drugs like orlistat, bupropion hydrochloride, naltrexone hydrochloride, and GLP-1RA, when non-medical interventions prove ineffective. Bariatric surgery is recommended for patients with a BMI > 40 kg/m2, those with a BMI of 35–39.9 kg/m2 and ≥ 1 obesity-related disease, and those with a BMI of 30–35 kg/m2 and uncontrolled type 2 diabetes mellitus. The paper also discusses the management of related conditions such as obstructive sleep apnoea, polycystic ovary syndrome, hyperuricaemia, sympathetic overdrive and tachycardia, and inflammation.
