TLDR Trichoscopy can reveal specific hair and scalp changes in linear morphea.
A 7-year-old boy with linear morphea alopecia presented with a progressive atrophic and slightly depressed linear plaque on the scalp, forehead, and left eyebrow. Trichoscopy revealed pinpoint white dots, loss of follicular openings, peripilar casts, pili torti, and short regrowing hairs. Histology showed epidermal atrophy, hyalinization, and widening of dermal collagen bundles. Despite no neurological or ophthalmological issues, systemic immunosuppressive therapy with prednisolone and methotrexate was initiated due to plaque progression. This report highlighted new trichoscopy findings in linear morphea, a form of localized scleroderma, marking the first detailed account of such features in this condition.
