TLDR Trichoscopy helps diagnose and manage different types of hair loss in the Indian population.
This narrative review emphasizes the significance of trichoscopy in diagnosing and managing alopecia in the Indian population. Trichoscopy aids in distinguishing between non-scarring and scarring alopecia by identifying specific hair and scalp features, such as hair diameter variability in androgenetic alopecia and exclamation mark hair in alopecia areata. It is particularly useful in differentiating conditions like chronic telogen effluvium and female pattern hair loss, guiding treatment decisions, and reducing the need for invasive biopsies. The review also notes the limitations of trichoscopy, such as a steep learning curve, and advocates for increased training among Indian dermatologists to enhance diagnostic and treatment outcomes.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

The conversation is about the role of testosterone in hair loss and the effectiveness of different treatments. The conclusion is that DHT is the main culprit for hair loss, and finasteride has been proven to work long-term in maintaining and regrowing hair. Testosterone may have a minor effect, but it is not the primary cause of hair loss.
