Epithelial Keratin and Filaggrin Expression in Seborrheic Keratosis: Evaluation Based on Histopathological Classification

The study investigated the expression of keratin and filaggrin in different histopathological types of seborrheic keratosis (SK) to understand their histogenetic origins. It found that hyperkeratotic SK expressed K1, K10, and filaggrin, indicating differentiation towards squamoid terminal keratinization. In contrast, acanthotic, irritated, clonal, reticulated, and adenoid SK mainly expressed K14, suggesting differentiation towards basaloid undifferentiated cells. Acanthotic SK showed differentiation towards the hair bulge, while irritated SK differentiated towards the follicular infrainfundibulum. The study concluded that SK is in an undifferentiated and hyperproliferative state with heterogeneous differentiation, and that immunohistochemical analysis of keratin expression is useful for the differential diagnosis of cutaneous epithelial tumors.
