Primary neutrophilic cicatricial alopecias, including dissecting cellulitis of the scalp, were characterized by scalp inflammation leading to scarring alopecia, with clinical presentations such as papules, nodules, pustules, and abscesses. Diagnoses like folliculitis decalvans and acne keloidalis nuchae showed overlapping symptoms, but trichoscopy and histopathology were crucial for differential diagnosis and staging. Early intervention was essential to prevent progression and cicatricial alopecia.
