Bicalutamide, a non-steroidal antiandrogen, shows potential in treating female pattern hair loss (FPHL) by inhibiting androgen binding to receptors without significantly affecting hormonal levels. A study involving 316 patients demonstrated its effectiveness at doses of 10-50 mg daily, both as a standalone treatment and in combination with other therapies like minoxidil and spironolactone. It has a favorable safety profile compared to other antiandrogens, with the most common side effect being an increase in serum transaminases, which usually resolves without dose adjustment. Bicalutamide may also help reduce minoxidil-induced hypertrichosis, suggesting its valuable role in FPHL management, though further research is needed to optimize its use.
