TLDR Renal transplant patients on tacrolimus and sirolimus may develop acne keloidalis nuchae.
The document reported two cases of acne keloidalis nuchae (AKN) in renal transplant patients receiving tacrolimus and sirolimus. The first patient, a 52-year-old Caucasian man, developed AKN one year post-transplant, while the second patient, a 27-year-old mixed-race woman, developed AKN six months post-transplant. Both patients showed improvement with intralesional dexamethasone treatment. The study highlighted the increasing reports of drug-related AKN in organ transplant recipients, particularly those on calcineurin inhibitors like cyclosporin, though the exact pathogenesis remains unclear. This was the first report of AKN in patients on tacrolimus and sirolimus, and the first involving a female patient, emphasizing the need for close monitoring of immunosuppressed transplant patients to better understand AKN's pathogenesis.
