A 26-year-old man with a 4-year history of folliculitis decalvans (FD) that was resistant to various treatments experienced significant improvement after 25 months of systemic photodynamic therapy (PDT) using ultraviolet light and porfimer sodium. This case highlighted the potential of systemic PDT, which has strong antibacterial effects, local immunomodulation, and improved scar healing, as a viable treatment option for recalcitrant FD with minimal adverse effects. This was the first report of using systemic PDT for FD.
