    TLDR Triple therapy with clobetasol, hydroxychloroquine, and N-acetylcysteine is recommended for better outcomes in treating lichen planopilaris.
    This systematic review and Bayesian network meta-analysis evaluated medical therapies for lichen planopilaris (LPP), a chronic disorder causing scarring alopecia. Seven studies with 251 patients were analyzed. The combination of clobetasol with N-acetylcysteine or pentoxyfilline showed significantly better outcomes in reducing LPP activity index compared to clobetasol alone. Conversely, cyclosporine, methotrexate, and methotrexate plus prednisolone were less effective than hydroxychloroquine. The study suggests a triple therapy of topical clobetasol, hydroxychloroquine, and N-acetylcysteine as the most effective treatment, while advising against less effective options like pioglitazone, mycophenolate mofetil, and cyclosporine for non-responders.

      A young female diagnosed with Lichen planopilaris (LPP) experienced years of misdiagnosis and ineffective treatments, including a hair transplant and other procedures. She is considering litigation due to the misdiagnosis and has learned that LPP is chronic, hair loss can be stopped with ongoing medication, but lost hair cannot regrow.
