TLDR Triple therapy with clobetasol, hydroxychloroquine, and N-acetylcysteine is recommended for better outcomes in treating lichen planopilaris.
This systematic review and Bayesian network meta-analysis evaluated medical therapies for lichen planopilaris (LPP) by analyzing seven studies with 251 patients. The study found that the combination of clobetasol with N-acetylcysteine or pentoxyfilline significantly improved the lichen planopilaris activity index (LPPAI) compared to clobetasol alone. Conversely, treatments with cyclosporine, methotrexate, and methotrexate plus prednisolone were less effective than hydroxychloroquine. The authors recommend a triple therapy approach using topical clobetasol, hydroxychloroquine, and N-acetylcysteine for better outcomes, while advising against the use of pioglitazone, mycophenolate mofetil, and cyclosporine unless other treatments fail.
