TLDR The best treatment for lichen planopilaris is a combination of clobetasol, hydroxychloroquine, and N-acetylcysteine.
This systematic review and Bayesian network meta-analysis evaluated the efficacy of medical therapies for lichen planopilaris (LPP) by analyzing seven studies with 251 patients. The study found that the combination of clobetasol with N-acetylcysteine or pentoxifylline significantly improved the lichen planopilaris activity index (LPPAI) compared to clobetasol alone. Conversely, treatments with cyclosporine, methotrexate, and methotrexate plus prednisolone were less effective than hydroxychloroquine. The authors recommend a triple-combined approach of topical clobetasol, hydroxychloroquine, and N-acetylcysteine for the most effective treatment, while advising against the use of pioglitazone, mycophenolate mofetil, and cyclosporine due to poor outcomes.
