Case Report of a Patient With Psoriasis Vulgaris, Psoriatic Arthritis, and Alopecia Universalis Successfully Treated With Tofacitinib and Methotrexate

    TLDR Tofacitinib and methotrexate successfully treated a woman's severe skin, joint, and hair loss issues.
    This case report details the successful treatment of a 31-year-old female patient with alopecia universalis, psoriasis vulgaris, and psoriatic arthritis using a combination of tofacitinib and methotrexate. Initially, the patient did not respond to various treatments, including methotrexate monotherapy and local therapies. However, after switching to a combination of tofacitinib and methotrexate, the patient experienced significant improvements, with her Severity of Alopecia Tool (SALT) score improving from 97 to 0, and her Psoriasis Area and Severity Index (PASI) score improving from 1.8 to 0. Additionally, her psoriatic arthritis showed clinical remission with a DAPSA score of 2. The report highlights the potential of tofacitinib, a Janus kinase inhibitor, in promoting hair regrowth and alleviating joint inflammation, suggesting it as a promising therapy for severe alopecia areata, though further large-scale studies are needed to confirm its long-term efficacy and safety.
