Does Imipramine and Pseudoephedrine Medical Treatment Have Clinical Implications in the Treatment of Retrograde Ejaculation Caused by Diabetes Mellitus?

    TLDR Imipramine and pseudoephedrine treatment can effectively restore normal ejaculation in patients with diabetes, increasing their chances for pregnancy.
    The study conducted between Jan 2015 and May 2017 investigated the efficacy of imipramine and pseudoephedrine dual therapy in 30 patients with retrograde ejaculation caused by diabetes mellitus. The treatment protocol included 25mg imipramine every 12 hours and pseudoephedrine 60mg every 8 hours daily for 2 weeks. Of the 18 complete retrograde ejaculation patients treated, 12 (66.6%) recovered spermatozoa in antegrade ejaculation with a mean total sperm count of 43.7±73.5 million. In the group with partial retrograde ejaculation, 6 out of 12 patients (50%) had a >50% increase in the antegrade total sperm count with a mean total sperm count of 54.4±60.4 million. The study concluded that imipramine and pseudoephedrine treatment is effective in establishing antegrade ejaculation in patients with diabetes mellitus, thus providing a chance for pregnancy either through intercourse or assisted reproductive technology.
