PSAT001 A Case of Ectopic Cushing's Syndrome Complicated by COVID-19 Infection

    TLDR A woman with ectopic Cushing's syndrome and COVID-19 passed away despite treatment.
    This case report discusses a 61-year-old female with ectopic Cushing's syndrome, complicated by a COVID-19 infection. The patient presented with symptoms including confusion, elevated blood sugars, and hypokalemia, and was found to have a right pulmonary mass, hepatic lesions, and adrenal gland thickening. Despite treatment with ketoconazole, metyrapone, and other supportive measures, her condition worsened after contracting COVID-19, leading to hypoxic respiratory failure. The use of IV steroids, despite her hypercortisolemia, did not improve her condition, and she eventually required intubation and hemodialysis. The patient's condition further deteriorated, leading to her passing. The report highlights the challenges in managing ectopic Cushing's syndrome complicated by COVID-19, particularly the unclear role of exogenous steroids in such cases.
