TLDR Ketoconazole improved symptoms of ACTH-independent Cushing syndrome despite inconclusive initial scans.
A 21-year-old woman with ACTH-independent Cushing syndrome presented with symptoms including hirsutism, secondary amenorrhea, and characteristic physical features. Initial adrenal CT scans were inconclusive, but later scans showed adrenal thickening. Laboratory tests confirmed the diagnosis, and treatment with ketoconazole improved her symptoms, including resumption of menstrual cycles and controlled blood pressure with spironolactone. The patient is considering surgical options in the future, and genetic studies are not available. The case highlights the challenges in diagnosing and managing ACTH-independent Cushing syndrome with inconclusive imaging results.
