Correlations Between Dermatology Life Quality Index and Trichoscopic and Clinical-Anamnestic Parameters in Ukrainian Men With Alopecia Areata

    M. A. M. Shakatira
TLDR Alopecia areata affects life quality differently based on its severity.
The study investigated the correlations between the Dermatology Life Quality Index (DLQI) and trichoscopic as well as clinical-anamnestic parameters in 81 young Ukrainian men with alopecia areata. The findings revealed that in the total group and those with grade II alopecia areata, there were only isolated weak correlations between DLQI scores and clinical-anamnestic parameters. In contrast, men with grade I alopecia areata showed multiple direct medium-strength, non-significant correlations with clinical-anamnestic parameters. Men with grade III alopecia areata exhibited multiple direct and inverse medium-strength, non-significant correlations with both trichoscopic and clinical-anamnestic parameters. The study highlights that the percentage of significant and medium-strength non-significant correlations was higher in men with grade I (16.28%) and grade III (37.21%) alopecia areata compared to the total group and those with grade II (9.30%).

How diffuse unpatterned alopecia (DUPA) is not an invitation to self-diagnose oneself with aggressive AGA and that seeking a specialized dermatologist may help people experiencing hair loss. Treatment options discussed include topical clobetasol propionate, oral minoxidil, and discontinuing finasteride.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

High DHEA levels may contribute to hair loss by increasing DHT in hair follicles, potentially explaining why finasteride is ineffective for some. Treatments like high-dose dutasteride and RU58841 are suggested, but the underlying cause, such as adrenal issues, should be investigated.

A 31-year-old man who previously had good results with minoxidil, finasteride, and nizoral for hair loss is now experiencing unusual hair loss and side effects from finasteride. Respondents suggest it might be severe retrograde alopecia and recommend exploring other treatment options due to his inability to tolerate finasteride.

The user noticed small circular hairless spots after a buzz cut and has been using finasteride, dutasteride, and oral minoxidil for hair loss. They are experiencing continuous shedding and thinning, and it is suggested they consult a dermatologist for a scalp examination and possible biopsy to determine if it's alopecia areata or androgenetic alopecia.
