TLDR Removing the ovarian tumor improved the woman's hormonal symptoms.
A 23-year-old woman presented with symptoms of androgen excess, including irregular menstrual cycles, amenorrhea, hirsutism, and significant weight loss. Her condition did not improve with oral contraceptives. Laboratory tests revealed elevated levels of DHEA-S, total and free testosterone, and 17-OH pregnenolone, as well as Inhibin B. Imaging showed a large ovarian mass, which was surgically removed and identified as an adult granulosa cell tumor. Post-surgery, her Inhibin B and testosterone levels decreased significantly, and her menstrual irregularities and hirsutism improved. This case highlights the impact of granulosa cell tumors on androgen levels and the effectiveness of surgical intervention.
