May 2025 in “Journal of the ASEAN Federation of Endocrine Societies” This case study describes a 66-year-old postmenopausal woman with significant hirsutism and alopecia, who was found to have a Leydig cell tumor in the left ovary, a type of androgen-secreting steroid cell tumor. Despite having primary hyperparathyroidism for 10 years, the patient developed symptoms of hyperandrogenism, including elevated testosterone levels and hair growth in atypical areas. After surgical removal of the tumor, her testosterone levels became undetectable, and facial hair growth reduced. The study highlights the importance of evaluating postmenopausal women with symptoms of hyperandrogenism for potential ovarian tumors, even when other conditions like hyperparathyroidism are present.
