TLDR An 81-year-old woman's severe male hormone symptoms were caused by an ovarian tumor, which was treated with surgery.
The document described a case involving an 81-year-old post-menopausal woman who presented with severe hyperandrogenism, including virilization and vaginal bleeding. Despite initial ultrasound results failing to detect an ovarian tumor, a CT scan later revealed a left ovarian solid nodule. Following a total hysterectomy and bilateral salpingoophorectomy, histopathology confirmed the presence of a Leydig cell tumor. After the surgery, the patient's androgen levels returned to normal, and the virilization symptoms improved, although clitoromegaly remained. This case highlights the need to consider virilizing ovarian tumors as a possible diagnosis in post-menopausal women with hyperandrogenism and related symptoms.
