TLDR Isotretinoin reduced inflammation in tufted hair folliculitis, but hair tufting remained.
Tufted hair folliculitis (THF) was a rare and challenging condition characterized by recurrent scalp folliculitis leading to scarring alopecia with multiple hair tufts emerging from dilated follicular openings. A 14-year-old female with a 10-year history of pruritic and inflammatory scalp wounds was diagnosed with THF. Initial treatment with oral rifampicin, cephalexin, and vitamin C was ineffective. However, isotretinoin treatment over six months reduced inflammation and exudation, although hair tufting persisted. This case highlighted the difficulty in treating THF and its rarity.
