Topical minoxidil solution (1% and 5%) can induce hair regrowth in alopecia areata, with a dose-response effect demonstrated. Patients with extensive scalp hair loss showed a higher response rate with 5% minoxidil. Occlusion of the treated area appears to be necessary to achieve and maintain maximum results. Increased T cell blastogenesis before treatment may predict response. Topical minoxidil offers advantages over other treatment modalities due to its simplicity and safety. Further studies on the efficacy of minoxidil in milder forms of the disease may be rewarding. No alterations in testosterone levels were found with the use of topical minoxidil.
