TLDR High potency topical steroids are the most effective treatment for pediatric alopecia areata.
This systematic review analyzed 23 studies involving 250 pediatric patients with alopecia areata (AA) to evaluate current treatments. The highest level of evidence (Grade 2) supported the efficacy of high potency topical steroids and 1% anthralin ointment, with significant hair regrowth and no additional side effects. Other treatments examined included the 308-nm excimer laser, zinc aspartate, biotin, clobetasol propionate, tofacitinib, methotrexate, and more, but these had lower levels of evidence (Grades 3 and 4). The review concluded that high potency topical steroids were the most effective, but larger clinical trials are needed to explore newer treatments for pediatric AA.
