TLDR SCAD can indicate ANA-negative lupus, especially in women with unusual symptoms.
This case report describes the first known instance of spontaneous coronary artery dissection (SCAD) as the primary presentation of ANA-negative systemic lupus erythematosus (SLE) in a 55-year-old woman. Despite negative ANA screening, the patient met the criteria for SLE diagnosis due to symptoms like hair loss, oral ulcers, and joint pain, along with positive anti-double stranded DNA antibodies. The case underscores the need to consider SCAD in myocardial infarction diagnoses, especially in women with atypical risk factors or rheumatological conditions, and suggests a low threshold for rheumatological evaluation in SCAD patients.
