Abstract 10845: Spontaneous Coronary Artery Dissection as the Primary Presentation of ANA-Negative Lupus

    TLDR SCAD can indicate ANA-negative lupus, especially in women with unusual symptoms.
    This case report describes a 55-year-old woman who presented with spontaneous coronary artery dissection (SCAD) during elective kidney donation, which led to a diagnosis of ANA-negative systemic lupus erythematosus (SLE). Despite negative ANA titers, the patient exhibited symptoms such as Raynaud’s phenomenon, hair loss, oral ulcers, joint pain, photosensitivity, malar erythema, and Sicca symptoms, and had strongly positive anti-double stranded DNA antibodies. This case underscores the importance of considering SCAD in myocardial infarction diagnoses, especially in women with atypical risk factors, and suggests a low threshold for rheumatological evaluation in SCAD patients.
