A 68-year-old postmenopausal woman with hyperandrogenism, presenting with increased facial hair and a deeper voice, was treated nonsurgically due to her poor surgical candidacy. Despite elevated testosterone levels and stable adrenal imaging, she was managed with Goserelin injections, which normalized her testosterone levels and improved symptoms. The case suggests that while bilateral oophorectomy is the definitive diagnostic and treatment approach for ovarian androgen secretion, GnRH agonists like Goserelin can effectively control symptoms and hormone levels in patients who cannot undergo surgery. The exact source of androgen excess, whether ovarian or adrenal, remains undetermined.
