A Case of Lupus Miliaris Disseminatus Faciei After Allogeneic Hematopoietic Stem Cell Transplantation

A 43-year-old man developed lupus miliaris disseminatus faciei (LMDF) after discontinuing cyclosporine A following an allogeneic hematopoietic stem cell transplantation. The patient presented with multiple reddish-brown papules on his face, which were confirmed through histopathological analysis to be epithelioid cell granulomas with caseation necrosis. Despite negative tuberculosis tests, the condition was treated effectively with a 6-month regimen of oral isotretinoin and topical tacrolimus, resulting in significant improvement. The case suggested that immunological changes post-transplantation might contribute to LMDF pathogenesis.
