Lichen Planus Pigmentosus and Lichen Planus Pigmentosus: 35 Cases in Armenia, Colombia

    Image of study
TLDR Lichen planus pigmentosus and fibrosing frontal alopecia in Colombia are likely different stages of the same disease.
The document presents a retrospective analysis of 35 cases of Lichen planus pigmentosus (LPP) and Fibrosing frontal alopecia (FFA) observed in Armenia, Colombia, from 2003 to 2014. The study included 34 women and 1 man, with a median age of 52 years, and the majority of patients were of Mestizo ethnicity, with phototypes III and IV. The patients primarily presented with pigmented grey changes on the face and neck, with 47% experiencing loss of eyebrows and 50% showing scalp involvement with alopecia along the frontal border. The average duration of the condition was 6.5 years. Histopathological examination revealed interface dermatitis in the epidermis and perifollicular at the isthmus level from the initial phase of LPP. The authors agree with Dlovac's proposal that LPP and FFA represent a spectrum of the same disease, beginning in photoexposed areas and later affecting the scalp in a pattern of fibrosing frontal alopecia. Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
