Altered Expression of Keratin 14 in Lesional Epidermis of Autoimmune Skin Diseases

TLDR Keratin 14 may be an autoantigen in autoimmune skin diseases.
The study investigated the expression of keratin 14 (K14) in autoimmune skin diseases (ASD) by analyzing skin samples from 58 patients and sera from 41 patients. It was found that K14 expression was retained in suprabasal layers of ASD-affected skin, with focal loss observed in the basal layer and hair follicles, particularly in conditions with interface dermatitis. Additionally, a scattered dot-like K14 staining was noted in the papillary dermis, especially in cutaneous lupus erythematosus and lichen ruber. The presence of anti-K14 autoantibodies in patient sera, as detected by Western blot, suggested that K14 might act as an autoantigen in ASD. The study concluded that the focal loss of K14 in the basal epidermis was associated with interface dermatitis and hair follicle involvement, indicating its potential role in the pathogenesis of ASD.
