Hederagenin, a pentacyclic triterpenoid saponin, exhibits significant therapeutic potential due to its anticancer, anti-inflammatory, antioxidant, and antimicrobial activities. It modulates cellular signaling pathways such as NF-κB, PI3K/Akt, and MAPK, inducing apoptosis in cancer cells and suppressing inflammation. However, its clinical application is hindered by poor bioavailability. Recent research focuses on chemical modifications, particularly at the C-28 position, to enhance its pharmacokinetic properties and cytotoxicity. Derivatives have shown improved potency and selectivity, with some achieving significant antiproliferative activity against cancer cell lines. Structural modifications and advanced delivery systems are crucial for overcoming pharmacokinetic barriers, and exploring interactions with the gut microbiome and potential applications in treating conditions like androgenetic alopecia could further enhance its therapeutic potential.
