Differential Gene Expression in Post-Finasteride Syndrome Patients

Skyler Howell, Weitao Song, Alexander W. Pastuszak, Mohit Khera
TLDR Gene expression differences in PFS patients suggest a potential organic cause for symptoms.
The study investigated gene expression differences in men with post-finasteride syndrome (PFS) compared to healthy controls, focusing on genes affecting neurosteroid levels and androgen receptor activity. It included 26 men with PFS symptoms and 26 controls. Results showed significant differences in gene expression, with 1,446 genes over-expressed and 2,318 under-expressed in PFS patients. Androgen receptor expression was notably higher in PFS patients. However, no significant differences were found in serum levels of androgen receptor activity markers or trinucleotide repeats. The study provided evidence of gene expression correlating with PFS symptoms, suggesting a potential organic etiology, but acknowledged limitations such as selection bias and lack of pre-exposure data.

Finasteride can impact neurosteroids, potentially causing depression and other side effects in some users. Despite these concerns, many continue using it for hair loss, with some switching to topical applications to mitigate side effects.

The conversation discusses the side effects of finasteride, including low libido and erectile dysfunction, and the possibility of these effects being permanent, known as post-finasteride syndrome (PFS). Some users report personal experiences with PFS and debate whether the condition is real, with varying opinions on the reversibility of side effects and the role of individual biology.
