TLDR A rare case of a man having both frontal fibrosing alopecia and vitiligo was reported.
A rare case was reported of a 58-year-old male patient with co-localization of frontal fibrosing alopecia and vitiligo. Frontal fibrosing alopecia is a scarring alopecia characterized by the recession of the frontotemporal hairline, primarily affecting postmenopausal Caucasian women and rarely men. The specific mechanisms of its development were still under study, with several hypotheses suggesting an association with other autoimmune disorders.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

The user experienced hair loss since 2019 and tried finasteride, dutasteride, RU58841, and ketoconazole shampoo without success. A scalp biopsy showed scarring and inflammation from folliculitis, indicating chronic inflammation was affecting treatment effectiveness.
