TLDR Finasteride 1.25 mg showed small improvements in hair density and thickness for female pattern hair loss, but more research is needed.
This study aimed to determine the efficacy of finasteride 1.25 mg on female pattern hair loss (FPHL). The study involved 18 normoandrogenic FPHL patients who were given finasteride 1.25 mg daily for 28 weeks. The treatment efficacy was evaluated with the changes of hair density and thickness assessed by phototrichogram and patients' and physician's global assessment using a 7-point scale score. After 28 weeks of treatment, phototrichogram assessment demonstrated a 5.87% mean increment of hair density and 11.8% mean increment of hair thickness without statistical significance. Ten of the 14 patients rated their states as 'no change' and the physicians also recorded 'no change' in 9 patients, at the end of the study. The study suggests that a dose higher than 1.25 mg/day, more number of patients, and longer duration of treatment are needed to confirm the effects of this clinical trial in normoandrogenic FPHL.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
