This editorial discusses the interaction between skin cells and immune cells in inflammatory skin diseases such as psoriasis and atopic dermatitis. It highlights the role of the epithelial-immune microenvironment (EIME), which includes microbial flora, barriers, epithelial cells, immune cells, and peripheral nerve endings, in providing defense and protective responses. Recent studies indicate that targeting specific biomarkers and immune signals with novel biologics and small molecule inhibitors is more effective and safer than traditional therapies. The review emphasizes the importance of understanding epidermal cells and their interactions to better comprehend the pathogenesis of these diseases, using models like atopic dermatitis, psoriasis, SLE, alopecia areata, and acne to illustrate disease pathologies.
