Reflectance Confocal Microscopy and Dermoscopy Analysis of a Case of Connective Tissue Nevi

Xiao Meng, Bingnan Cui, Yijia Xu, Ling Zhang
TLDR Connective tissue nevi have distinct features, and reflectance confocal microscopy is useful for early diagnosis.
The study investigated connective tissue nevi (CTN) using dermoscopy and reflectance confocal microscopy (RCM), analyzing pathological features based on lesion duration. For lesions over 1 year, dermoscopy showed distinct grey-white maculae with clear boundaries, surrounded by brown to black pigments, and various vascular and hair structures. RCM revealed weakened tissue refraction due to thickened epidermis. Lesions under 1 year displayed similar maculae but with increased dermal papillae density and brightness, forming a honeycomb structure. The study concluded that CTN has distinct dermoscopic and RCM characteristics, with RCM being particularly useful for diagnosing early lesions.

A 27-year-old male with diffuse hair loss, including the donor area, did not respond to finasteride, dutasteride, or minoxidil. He suspects his hair loss may be linked to a mild connective tissue disorder, possibly affecting the structural support of hair follicles, rather than being purely hormonal.

Dutasteride may cause chronic infertility by reducing semen volume and motility, with concerns about irreversible prostate fibrosis. Users consider switching to finasteride or using treatments like minoxidil and microneedling, weighing hair preservation against fertility.

Procapil is marketed as a natural hair loss treatment but lacks strong evidence and is industry-biased. Minoxidil and Finasteride are the only FDA-approved treatments for androgenetic alopecia.
