TLDR Fat tissue transplant may be an effective new treatment for severe, treatment-resistant hair loss from folliculitis decalvans.
In 2018, Marinella Tedesco reported on a case where autologous fat transplantation was used as a treatment for a female patient with recurrent folliculitis decalvans (FD), a scarring type of hair loss. The patient, who had suffered from FD for 25 years and was unresponsive to conventional treatments, underwent two adipose tissue transplants. Following the treatment, the patient showed no signs of new pustules, pain, or burning, and experienced hair regrowth in the affected areas, with the hair quality improving to be stronger and shinier. This case indicated that adipose tissue transplant might be an effective alternative therapy for severe and resistant FD, leading to clinical remission and reduced reliance on traditional therapy. However, the author emphasized the need for further studies with more participants and longer follow-up to confirm the treatment's efficacy.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

Why the top of the head is affected by hair loss more than other areas, and treatments such as surgery, medications like minoxidil, finasteride, and RU58841 to treat it.

Topical finasteride is considered a safer option for hair regrowth with fewer systemic side effects compared to oral finasteride. Some users report similar efficacy and side effects between topical and oral treatments, while others prefer topical due to reduced systemic exposure.
