TLDR Early recognition and treatment of severe SLE symptoms can improve outcomes.
A 15-year-old female patient was diagnosed with systemic lupus erythematosus (SLE) after presenting with symptoms including joint pain, facial redness, fatigue, and hair loss. She developed complications such as lupus nephritis, cerebritis, and diffuse alveolar hemorrhage (DAH), which is a rare but serious complication of SLE. DAH was identified through bronchoalveolar lavage and presented with symptoms similar to bacterial pneumonia, including hemoptysis, dyspnea, and desaturation. Treatment with methylprednisolone and cyclophosphamide led to significant improvement in her condition, including vision, dyspnea, and headache. The case highlights the importance of considering DAH in SLE patients with respiratory symptoms and the potential for improvement with appropriate treatment.
